What information do we hold and why?
We will only ask you for information relating to your care to enable us to provide you with accurate diagnoses and to facilitate treatment.
Information such as Name, D.O.B, contact details and medical history, treatment history, appointments and recalls.
Who will it be shared with?
We will ONLY share your information with people relating to your care, and ONLY the information needed to facilitate this.
How is it stored?
Paper records are stored in locked fireproof cabinets away from the public areas, Computer records are stored securely on password protected computers and backed up daily on and off site with a reputable IT company. (PS4B)
How long will we store it?
As long as you are a patient we will retain your records however should you cease to be a patient then we will retain your information for 15 years or for children until the age of 25 years, (whichever is the longer).
What if I don’t want my information stored?
You do not have to provide us with any information about yourself, however this may prevent us from carrying out a thorough diagnosis which may then have implications regarding treatment.
You maintain the ‘right to be forgotten’ at all times, this means that you can request that your records be deleted at any time. We are obliged by law to fulfil this request, however please note that we may then be unable to provide adequate diagnosis and/or treatment.
You may also request to see your records at any time (no charge), and you retain the right to complain.
How will we dispose of this information?
Paper records are destroyed using Datashred, a secure shredding company, Computer records will be permanently deleted.
